I understand that participation in soccer involves inherent risks of injury, including but not limited to sprains, fractures, concussions, and other physical harm. I voluntarily assume all such risks on behalf of my child.

I release Galveston Soccer Academy, its officers, volunteers, coaches, and affiliated organizations from any and all claims, damages, or liability arising from my child's participation in league activities, including practices, games, and team events.

I consent to emergency medical treatment for my child if I cannot be reached and treatment is deemed necessary by medical personnel. I accept financial responsibility for any such treatment.

I confirm that my child is physically able to participate in competitive soccer and has no medical conditions that would prevent safe participation, or that I have disclosed such conditions in the medical field above.
